This is an old question but the other answers didn't really answer your question.
There are several free ways to make a script safe from different types of tampering:
Policing - Don't allow the user to tamper the script or system in any way. This is usually not a viable solution though.
Making the code impervious to easy inspection. This is usually not possible in the case of PHP scripts.
Ensuring the origin of the file is safe and known, as most of the threats happen online these days. If you download the script from a valid source and are confident of the software you are using, the only way someone can tamper with it is by making their own, which is usually not possible.
It is difficult to answer this question because there are no standards that you can follow.

Run PHP's built-in auto_prepend_file ini setting, and configure it to use your own file as a pre-prepend to the beginning of all scripts:
auto_prepend_file = "./my-own-safe-script.php"

It's a good idea to have a separate script which is'responsible' for safe-ifying the main script.
